You may register up to five people on the same application. You will have the opportunity to pay your deposit of $200 per person via PayPal when you apply online.  Please note that your application will not be complete until your deposit has been received.

Application Form – If you prefer, you may submit a paper application form.  To do so, please download the 2012 Application Form,  complete it and send it to the Folk Arts Center office, 10 Franklin St., Stoneham, MA 02180, by mail or email.  Our email address is fac@facone.org.

Youth Forms are required for campers under 18. They are due to the Folk Arts Center office by mail or email by July 2, 2012.

2012 Fees

Standard fees include lodging in double rooms, all meals, and tuition. A $40 discount per person is given for multiple occupancy (rooms with three to six people), subject to availability. Please note your preference for multiple occupancy on the application form if you want the discount.  Availability is limited and we cannot guarantee multiple occupancy.

Folk Arts Center members receive a $25 discount off their camp fees.

Student (age 13+) (double occupancy) $910
Young Student (age 5-12, double occupancy) $780
Companion (double occupancy) $780

No part-time campers, please.  Companions may audit but not play in classes.

Applications received by Thursday, March 15 will be given first priority. Applications received after that date will be assigned priority by the date of receipt (for mailed applications, we will use the postmark date).  Dorm rooms and class space are limited and camp fills quickly. Please plan accordingly.

Deposit, Cancellation and Refund Policy

A deposit of $200 per person is payable with the application.  If paid by check, the deposit will be cashed upon acceptance to the school.  If paid by credit card or PayPal, your deposit will be promptly refunded if you are not admitted to BHSFS.  If cancellation is necessary, $150 of the deposit is refundable through June 15, after which it is nonrefundable. Payment in full of all camp fees is due by June 15. After July 2 there will be no refunds of camp fees; however, you may receive credit for a tax-deductible donation to the Folk Arts Center.

Work Exchange

A limited number of partial work exchange positions are available for campers who are at least 18 years old. The average work exchange award is $150.  Indicate that you are interested in a work-study position on the application form and we will contact you with more information.

Payment

You can use this worksheet to calculate the balance of your camp fees that are due by June 15.

Younger Campers

The parent(s) of each under-16 camper must appoint another adult, who will be present at camp, to be responsible for their child while at BHSFS if they will not be present themselves.  They must do so in writing by completing and signing the Responsible Adult Form, which must also be signed by the adult they are appointing.  Please complete it and send it to the Folk Arts Center office, 10 Franklin St., Stoneham, MA  02180 by mail or email by July 2, 2012.  Our email address is fac@facone.org.  If you need help finding an adult to be responsible for your child at camp, the office will be glad to assist.  Please call Marcie at the office, 781-438-4387.

In addition, all under-18 campers must submit a health form. Please complete it and send it to the Folk Arts Center office, 10 Franklin St., Stoneham, MA  02180, by mail or email by July 2, 2012.  Our email address is fac@facone.org.
